Wellness Inventory - Assignment 2

Before we can start setting goals we need to start with a baseline.  Over the next couple of weeks we will be collecting a variety of baseline information.  To start, you will be completing a Wellness Inventory.  This document can be located in your Classes folder under Wellness 10.  The document you will be working on is titled Wellness Inventory 2013.  You can complete the inventory in Word by placing an "X" in the box or you can print it off and complete it with a pen/pencil.  You will have to print it off eventually to complete your wellness wheel and the questions.  I have also put a sample in your folder for you to look at titled, Wellness Inventory Example.  When looking at the example you will notice that the last 4 pages are justification for all items that I selected a 4 or 5 for.  You need to justify all the items that you check off that are greater than 3.  If you are unsure what to do, make sure to check the example.  This should take you quite some time if you are doing this properly.  The inventory took me 45 minutes to complete and my justifications took an hour for just the Physical Dimension section.  Make sure to read all the instructions on the Inventory.  You will also need a calculator and 5 different colours of pencil crayons to complete this.  No one else except me will see your inventory so please be honest.  Sometimes the truth hurts but we cannot improve if we do not first acknowledge the problem.  This a great personal learning tool and lots of fun (or at least it was for me!).  I have challenged some other teachers to complete it as well!

    I hope you're enjoying the break! I finally got a chance to look at your Wellness Inventories and will enter your marks in later today or tomorrow. Just to let you know that your inventory was out of 45 marks. 25 for completing the inventory, 5 for properly colouring in the wheel, 5 marks for the questions and 10 marks for justifications. If you received a "0" it's because you have no yet handed in the assignment.
